To get to know you better, there is some paperwork I'd like you to fill out before your first session. You may download the forms from our website by clicking on the links.
Step 1—Read and save the "Welcome document"
Step 2—Print and bring the "Information Requested" document to our first meeting.

In order to read and print these documents you'll need a pdf reader. I recommend Foxit Reader for its small size and fast loading, but Adobe Reader works as well. If you own a Mac, the document will open in Preview.
Payment:
I accept cash, checks and credit cards through Paypal. Click on the button below to make your payment with Paypal or a credit card. When you get to the payment page it will state "Make a Donation." I've set that up to accommodate different fee structures. Please enter the amount you have arranged with the therapist.
